Services
Ultramist® is an FDA-cleared, non-contact low-frequency ultrasound technology used for advanced wound care. It promotes healing by delivering therapeutic ultrasound waves through a gentle saline mist directly to the wound bed. This technology accelerates tissue regeneration, reduces bacteria, and improves blood flow to the wound site — all without direct contact, making it comfortable for patients.
RPM uses FDA-approved medical devices to track your vital signs from home. Depending on your condition, you may receive a blood pressure monitor, pulse oximeter, glucometer, or weight scale. These devices automatically transmit your readings to our clinical team, who review your data daily. If any readings fall outside your personalized thresholds, our team contacts you promptly to address the issue before it becomes an emergency.
CCM is available to patients who have two or more chronic conditions expected to last at least 12 months. Common qualifying conditions include diabetes, hypertension, heart failure, COPD, chronic kidney disease, arthritis, depression, and obesity. Your care coordinator will work with you to create a comprehensive care plan and coordinate all aspects of your treatment.
Transitional Care Management (TCM) is a service designed to ensure a safe and seamless transition from a hospital, skilled nursing facility, or rehabilitation center back to your home. Our team contacts you within 24 hours of discharge, schedules a follow-up visit within 7 to 14 days, reconciles your medications, and coordinates with your specialists to prevent hospital readmissions.
Yes. Sterling Health Solutions partners with hospitals, skilled nursing facilities, and physician practices across the Dallas-Fort Worth Metroplex. We receive real-time ER encounter notifications, enabling us to provide rapid follow-up care for discharged patients. Our hospital partners benefit from reduced readmission rates and improved patient satisfaction scores.
Insurance & Billing
Yes, we accept Medicare Part A & B, Medicare Advantage plans, and Medicaid/CHIP. Many of our services, including wound care, RPM, CCM, house calls, and transitional care management, are covered under Medicare with minimal or no out-of-pocket cost to qualifying patients.
We accept Medicare Part A & B, Medicare Advantage, Medicaid/CHIP, Blue Cross Blue Shield, Aetna, Cigna, UnitedHealthcare, Humana, and most private insurance plans. If you are unsure whether your plan is accepted, please contact our office and our team will verify your coverage at no charge.
For Medicare-eligible patients, RPM devices are typically provided at no cost. The devices remain the property of Sterling Health Solutions and are loaned to you for the duration of your monitoring period. Setup, training, and ongoing data monitoring are all included as part of the RPM program and are billed directly to your insurance.
