Wound Care
Medicare Part B covers wound care services, including Ultramist® therapy, when deemed medically necessary. Most private insurance plans also provide coverage for wound care treatment.
Remote Patient Monitoring
RPM is covered under Medicare for patients with qualifying chronic conditions. Medicare covers device setup, patient education, and ongoing monitoring. Most Medicare Advantage plans also provide coverage.
Chronic Care Management
CCM is covered under Medicare Part B for patients with two or more chronic conditions. Patients are typically responsible for a minimal copay (approximately 20% of the Medicare-approved amount). Many supplemental plans cover this copay.
House Call Primary Care
Medicare covers home visits for patients who are homebound or have difficulty traveling to a clinic. Coverage includes evaluation and management services, medication management, and care coordination.
Transitional Care Management
TCM is covered by Medicare with no patient copay for qualifying discharges from hospitals, skilled nursing facilities, and other inpatient settings. Coverage includes the initial contact, follow-up visit, and care coordination.
